As of the fall of 2022, our office has joined the Alberta Facilitated Access to Specialized Treatment (FAST) Program. Please update your outbound fax numbers to include the Alberta FAST office toll-free fax number 1-833-627-7024 which can be used for all non-urgent orthopedic referrals.

 

FAST is open Monday-Friday from 7:30am-6pm and can be reached by phone at 1-833-553-3278 ext. 5 (South Zone).

The Hip and Knee Replacement Referral Form is still accepted by FAST, as is the FAST referral form with some additional information required for orthopedic referrals.
