Referrals to Chinook Bone and Joint Clinic

As of the fall of 2022, our office has joined the Alberta Facilitated Access to Specialized Treatment (FAST) Program. Please update your outbound fax numbers to include the Alberta FAST office toll-free fax number 1-833-627-7024 which can be used for all non-urgent orthopedic referrals.

 

FAST is open Monday-Friday from 7:30am-6pm and can be reached by phone at 1-833-553-3278.

Chinook Bone and Joint Clinic requires one of the following forms/letters:

• The FAST Non-Urgent Adult Orthopedic Referral.

• The legacy Hip and Knee Replacement Referral Form.

• A letter from the primary care provider outlining the affected joint(s) and history of OA symptoms and attempted treatments

 

Please ensure the patient’s updated demographics are included with every referral.
